Which questions go on the application form, a custom form, or feedback?

This article is for group managers. An opportunity can have three forms. Put each question on the form that matches when you need the answer, and what you will do with it.

Ask this before you add a question: if you removed it, could you still approve the right people?

  • If no, it belongs on the application form.
  • If yes, and you only need it from people who are coming, use a custom form.
  • If yes, and you only need it after the opportunity is over, use feedback.

Application form

Volunteers answer this when they apply. Use it when the answer helps you accept or decline them, or when you need it from everyone who applies.

Examples:

  • Do you have a Working with Children Check?
  • Who is your emergency contact?
  • Do you have a driver's licence? (when the role requires driving)

Custom form

Only approved volunteers answer this. You do not use it to accept or decline anyone. You can create more than one, and send each one to the people who need it. A signature, such as a waiver or photo consent, goes on a custom form.

Examples:

  • How will you travel to the site on the day?
  • What shirt size should we order?
  • Can you bring your own gloves?
  • Please sign this photo consent.

Feedback

Volunteers answer this after they have taken part. It is about how the opportunity went, not about whether they can attend. Each opportunity has one feedback survey.

Examples:

  • How satisfied were you with this opportunity?
  • Would you volunteer with us again?
  • What went well, and what should we change?

Dietary needs

  • You cannot place someone on a cooking shift unless you know what they can handle. Put it on the application form.
  • Everyone who is coming will be fed, and you only need the answer to order the right food. Put it on a custom form after you approve them.
  • You want to know whether the food on the day was suitable. Put it on feedback.

Travel

  • The role requires a driver. "Do you have a driver's licence?" goes on the application form.
  • You are arranging lifts for people already approved. "Will you drive or take the train on Saturday?" goes on a custom form.
